Please fill out this form to let the library know the types and number of materials you would like us to select for you.

Within two business days, a staff member will curate a selection of items just for you!You will receive automated notification(s) when your item(s) are ready. You may then come into the library to pick them up, or call  us at 608-588-2276 to schedule curbside service.

How do you plan to retrieve your items?

There are three boxes below in which to choose items for children, teens, and adults. Families may use one form to submit their requests. 

Request Materials for Children
The more details the better, like the type of book, favorite authors or other titles, fiction or non-fiction, etc. If it doesn't matter, and you'd just like us to choose a certain number from given collection(s), that's fine too (i.e., 15 board books, 10 easy readers, five graphic novels).
Is there anything you'd like to add about what you are *not* looking for (i.e.,authors/titles your child(ren) has/have already read)?

What type of items are you looking for? The more details the better, like the genres you like (i.e., murder mysteries, memoirs, non-fiction), favorite authors or titles, length of the book, any format preferences (large print, audiobook), etc.
